The Colorado Clinical Therapy, LLC website is intended to provide general educational information and to help guests more easily arrange for health care services. The CCT website is not an attempt to practice medicine or therapy or to provide specific medical or therapeutic advice and should not be used to make a diagnosis or to replace or overrule a qualified healthcare provider’s judgment. Nor should guests rely upon the CCT website if they might need emergency medical treatment. We strongly encourage guests to consult with a qualified healthcare professional for answers to personal questions.
Use of programs on the CCT site does not establish a doctor-patient relationship. Should you electronically request a referral to a CCT therapist, we will use the information you submit to arrange for care where appropriate. You assume full responsibility for using the information on this site, and you understand and agree that CCT and its affiliates, are not responsible or liable for any claim, loss, or damage resulting from its use by you or any user. While we try to keep the information on the site as accurate as possible, we disclaim any warranty concerning its accuracy, timeliness, and completeness, and any other warranty, express or implied, including warranties of merchantability or fitness for a particular purpose. This disclaimer applies to information not only in the English version but also in other languages translated by the site translator. You may use the CCT website without disclosing personally identifiable information. We want to be clear that we will not obtain such information about you unless you choose to submit it to us. For example, if you email us your comments, that identifies your email address. If you request an appointment with a physician, a limited number of hospital administrators and healthcare providers will see this information and use it to arrange care where appropriate.
We have taken reasonable steps to ensure the integrity and confidentiality of personally identifiable information that you may provide. You should understand, however, that electronic transmissions via the Internet are not necessarily secure from interception, and we cannot guarantee the security or confidentiality of such transmissions.
The web server of the CCT website collects and saves the default information customarily logged by World Wide Web server software. Our logs contain the following information for each request: date and time, originating IP address and domain name (the unique address assigned to your Internet service provider’s computer that connects to the Internet), object requested, and completion status of the request.
Except for authorized law enforcement investigations or other valid legal processes, we will not share any information we receive with any parties outside of Colorado Clinical Therapy, LLC.

Colorado Clinical Therapy, LLC is committed to providing you with quality healthcare and to form a relationship with you that is built on trust. That means respecting the privacy and confidentiality of your medical information. We practice policies and procedures that allow access to your personal medical information only for purposes related to your medical care.
We are required to maintain a complete record of your medical history, current condition, treatment plan, and all diagnosis and treatment given, including the results of all tests, procedures, and therapies. Whether this information is stored in writing, on a computer, or other means, we will keep this information in a safe and secure way that protects your privacy and confidentiality. Of course, the physicians and other health care professionals who are involved in your care need to access this information to provide appropriate treatment for you.
You, or anyone to whom you give permission, or a legally designated representative, have the right to read or get a copy of your medical information. Your medical record is the physical property of the individual hospital or physician practice.
Colorado Clinical Therapy, LLC has put in place detailed policies regarding access to medical records by our staff and employees and has carefully outlined the circumstances under which your medical information may be released to parties outside of the CCT practice. These policies conform to state and federal law and are designed to safeguard your privacy. Our staff and employees are trained in the appropriate use of your protected health information (medical information) and know that it is available to them only to continue to provide care to you, obtain payment for care and for operations or for other limited but legitimate reasons. A violation of confidentiality or the failure of an employee to protect your information from accidental or unauthorized access will not be tolerated. This may include the employee being fired from his or her job. Failure to keep your information confidential could result in actions up to including an employee being fired from his or her job.
We do not allow others to access your medical information, unless we have the appropriate authorization or other legal basis to do so. Some laws prevent certain types of patient information from being released without specific patient permission. Examples include, but are not limited to, communications between patient and psychotherapist or social worker, sexually transmitted disease test results or visit notes, HIV tests and related information, substance abuse rehabilitation treatment records, and sexual assault treatment records.
This includes mandatory reports of abuse of children, elderly people, or persons with disabilities. Medical records are sometimes used for reasons other than patient care. Records are periodically reviewed to evaluate the quality of care, or to be sure that the Colorado Clinical Therapy, LLC organizations follow the rules of regulatory agencies such as Medicare or the Departments of Mental and of Public Health. Anyone reviewing records must follow the same confidentiality laws and rules required of all healthcare providers. Patient records are valuable tools used by researchers in finding the best possible treatments for diseases and medical conditions.
Public Health Activities We may disclose your Protected Health Information or PHI for the following public health activities: (1) to report health information to public health authorities for the purpose of preventing or controlling disease, injury or disability; (2) to report child abuse and neglect to the Colorado Department of Children and Family Services or other government authorities authorized by law to receive such reports; (3) to report information about products and services under the jurisdiction of the U.S. Food and Drug Administration; (4) to alert a person who may have been exposed to a communicable disease or may otherwise be at risk of contracting or spreading a disease or condition; and (5) to report information to your employer as required under laws addressing work-related illnesses and injuries or workplace medical surveillance.
Researchers must also follow strict HIPAA rules to ensure the privacy of patient information and. information that may identify you. In all research conducted by Colorado Clinical Therapy, LLC, concern for your privacy and your well-being is our first priority. Uses and Disclosures for Treatment, Payment and Health Care Operations We may use and disclose Protected Health Information or PHI in order to treat you, obtain payment for services provided to you and conduct our “health care operations” as detailed below:
- Treatment We use and disclose your PHI to provide treatment and other services to you, i.e., to diagnose and treat your injury or illness. In addition, we may contact you to provide appointment reminders or information about treatment alternatives or other health-related benefits and services that may be of interest to you. We may also disclose your PHI to other providers, including consulting physicians, who are not employees of Colorado Clinical Therapy, LLC, involved in your treatment.
- Payment We may use and disclose your PHI to obtain payment for services that we provide to you. For example, we may use your PHI for disclosures to claim and obtain payment from Medicare, Medicaid, your health insurer, HMO, or other company or program that arranges or pays the cost of some or all of your health care, or to verify that your payor will pay for health care.
- Colorado Clinical Therapy, LLC’s Health Care Operations We may use and disclose your PHI for our health care operations, which include internal administration and planning and various activities that improve the quality and cost-effectiveness of the care that we deliver to you. For example, we may use PHI to evaluate the quality and competence of our physicians, nurses and other health-care workers. We may access your PHI in order to resolve any concerns you may have and ensure that you have a comfortable visit with us.
For Further Information; Complaints If you desire further information about your privacy rights, are concerned that we have violated your privacy rights or disagree with a decision that we made about access to your PHI, you may contact our Office at firstname.lastname@example.org. You may also file written complaints with the Director, Office for Civil Rights of the U.S. Department of Health and Human Services. Upon request, the Colorado Clinical Therapy, LLC Office will provide you with the correct address for the Director. We will not retaliate against you if you file a complaint with us or the Office for Civil Rights.
Right to Request Additional Restrictions You may request restrictions on our use or disclosure of your PHI: (1) for treatment, payment and health care operations; (2) to individuals (such as a family member, other relative, close personal friend or any other person identified by you) involved with your care or with payment related to your care; or (3) to notify or assist in the notification of such individuals regarding your location and general condition. While we will consider all requests for restrictions carefully, we are not required to agree to a requested restriction unless the disclosure is to a health plan for purposes of carrying out payment or health care operations, and the information pertains solely to a health care item or service for which you have paid us out of pocket in full. If you wish to request additional restrictions, please obtain a request form from our Office and submit the completed form to the Office. We will send you a written response.
Right to Receive Confidential Communications You may request, and we will accommodate, any reasonable written request for you to receive your PHI by alternative means of communication or at alternative locations.
Right to Revoke Your Authorization You may revoke Your Authorization, or any written authorization obtained in connection with your PHI, except to the extent that we have taken action in reliance upon it, by delivering a written revocation statement to the Colorado Clinical Therapy, LLC Office. A form of Written Revocation is available upon request from the Office.
Right to Inspect and Copy Your Health Information Timely You may request timely access to your medical record file and billing records maintained by us in order to inspect and request copies of the records. Under limited circumstances, we may deny you access to a portion of your records. If you desire access to your records, please obtain a record request form from the Colorado Clinical Therapy, LLC Office and submit the completed form to the Office. If you request copies, we may charge you for each page. We may also charge you for our postage costs, if you request that the copies be mailed.
Right to Amend Your Records You have the right to request that we amend PHI maintained in your medical record file or billing records. If you desire to amend your records, please obtain an amendment request form from the Colorado Clinical Therapy, LLC Office and submit the completed form to the Office. We will comply with your request, unless we believe that the information that would be amended is accurate and complete or other circumstances apply.
Right to Receive an Accounting of Disclosures Upon request, you may obtain an accounting of certain disclosures of your PHI made by us during any period of time prior to the date of your request provided such a period does not exceed six (6) years and does not apply to disclosures that occurred prior to April 14, 2003. If you request an accounting more than once during a twelve (12) month period, we may charge you for the accounting statement.
Right to Receive Paper Copy of this Notice Upon request; you may obtain a paper copy of this Notice, even if you have agreed to receive such notice electronically.
